Long Term Housing & Navigator
Program Manager

she/her/hers

Amy Jacobs has always felt a desire to help improve the lives of others. Amy began working towards this goal, while still a student, by joining the staff at Magdalene Home where she was then recruited to join OnTrack’s Home Program. She worked for ODHS/Child Welfare Office as a member of the Addiction Recovery Team (ART), then became the ART team lead. She earned her CADC and trains others to increase their knowledge of substance use disorders and identification of substance use, and the nexus between ongoing substance use and child safety. In 2015, Amy began as the supervisor of all housing staff and Community Family Court Supervisor for OnTrack. In February 2017, she joined the Family Nurturing Center to help create the Long-Term Housing Program, encouraging its growth including expansion to Josephine County.

Amy was born and raised in Southern Oregon and has a deep love for horseback riding.

Amy continues to have the same passion for work after 15 years serving the vulnerable populations in Jackson and Josephine Counties. She now supervises multiple Child Welfare contracts and still has a love for teaching new staff and helping create long term sustainability and change for vulnerable families.
